How It Works
At TEXAS CARE DIALYSIS, we provide the highest quality and personalized in-house home hemodialysis treatments. Texas Care Dialysis creates a better coordinated care with constant communication with the SNF team and with our monthly Quality Assurance and Performance Improvement assessment to address all renal concerns.
We will work as a team with the Skilled Nursing Facility (SNF) staff, the patient and the patient’s Nephrologist to address hemodialysis patients’ care as a whole.
Onsite dialysis with Texas Care may help reduce the stress associated with being transported back and forth three times a week to outpatient dialysis clinic.
